Signed before they sit down.
New clients fill in their details and agree to your waiver at home, not on a clipboard at the front desk. Every intake is saved with the date, and the details you should check are flagged for you.
It starts from“An intake page for my studio where new clients make an account, fill in their details, upload an ID, agree to my waiver by typing their name, it’s saved with the date, AI flags anything I should check, I get an email with each intake, clients sign in later to see what they’ve signed, and then they go on to my booking page.”

What comes working in a client intake app?
The clipboard, off the front desk.
The parts a first visit runs on come working in the first version, with your questions, your waiver and your logo.
- ✓ Comes working01
Your intake questions, your words
Contact details, the reason for the visit, anything you need to know first. Every answer saved to the client’s file.
- ✓ Comes working02
A waiver agreed by typed name
Your waiver text, an “I agree” box and their typed name, saved with the date and time they agreed.
- ✓ Comes working03
ID and insurance uploads, kept private
Clients add a photo of an ID or an insurance card. Only you can open it, and it never shows on a public page.
- ✓ Comes working04
Flags on anything to check
AI reads each intake and flags what needs a look before the visit, like an allergy, an injury or a question left blank.
- ✓ Comes working05
Clients sign in to see what they signed
Clients make an account as they start, then log in any time to see their own intake and waiver, with the date. Nobody else’s.
- ✓ Comes working06
A client list in your Admin
Every intake in a table in your Admin, with the client’s name, the flags and the waiver date on each row.

On to booking, next
When the intake is done, clients land on your booking page, so the next step is picking a time.
What new clients fill in, and what you check.
Clients answer your questions, agree to your waiver and upload an ID. You get a client list in your Admin, with the details worth checking flagged on each intake.
How does online intake save a studio or clinic time?
Start on time, not on paperwork.
A paper intake eats the first ten minutes of a first visit. The client fills in a clipboard in the lobby, you read it standing up, and the form goes into a folder you will never find again. Online intake moves all of that out of the lobby: they fill it in at home before they come, you read the flags over coffee, and the session starts on time.
The waiver stops being a pile of paper too. Every agreement is saved with the date and a typed name, next to the client it belongs to.
- On timeThe paperwork is done before they walk in.
- Read the flagsInjuries and allergies flagged before the visit.
- Find it laterEvery waiver on file with the client’s name and the date.
It records a typed-name agreement with the date, not a certified e-signature, and it isn’t set up for medical records that need HIPAA compliance.
